If so, then you are a carer and are entitled to support to help you in your caring role. This can include 1 to 1 advice, support groups with other carers, training, and help with costs.


Here at Glenside Country Practice we are committed to making sure carers are given the chance to obtain any support they feel they need, be it simply information and advice to emotional support and benefits.


If you would like to know more, the Practice has 2 Carer Champions, 1 at each of our sites – Michelle at Corby Glen and Dawn at Castle Bytham - who can give you further information, or you can call the Lincolnshire Carers Service at the County Council Customer Service Centre (CSC) on:

Both numbers are available Monday to Friday 8am – 6pm


Carers FIRST is an organisation which offers emotional and practical support to carers, and after your initial contact with CSC you can be referred onto them for further assistance.


It is important to know that you, as a carer, can have the support you need.

Glenside Country Practice

With patient’s needs at the heart of everything we do, our website has been designed to make it easy for you to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register, you’ll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations. 

Extended Access Appointments

We are pleased to be able to offer our patients access to routine, pre-booked appointments at evenings and weekends from Monday 10th September.

The new service is part of an extended access service which has been commissioned by NHS South West Lincolnshire Clinical Commissioning Group.

Extended hours run between 18:30 and 20:00 weekdays and 09:00 and 12:00 Saturday and Sunday and are available on bank holidays and across the Easter, Christmas and New Year periods.

Appointments are for pre-bookable, non-urgent consultations with GPs and Nurses and Physiotherapists.

The service is being delivered at the St Peter’s Hill Surgery and managed by a federation of GPs in the Grantham area. This means that your appointment could be with a health professional from any of the federation practices.

If you, or someone you know, are an unpaid carer, there is support available. Please see the noticeboard at the top of the page for more information or get in touch with us.

Patient Participation Group

The annual patient participation group report is now availabe in the Patient Participation Group section of the website.

Named GP for over 75s

New service for patients

 

As of April 2015 all practices are required to provide all their registered patients aged 75 and over with a named GP, who will have overall responsibility for their care and support.

 

We will write to all patients in this age group informing them of their named GP.

 

This does not prevent you from seeing any GP or Nurse in the practice as you currently do.

 

Accountable GP

All patients registered at the practice, will be assigned an accountable GP.  This does not prevent you from seeing any GP in the practice as you currently do.

Accessible Information Standard

This standard aims to ensure that people who have a disability, impairment or sensory loss are provided with information that they can easily read or understand with support so they can communicate effectively with services.

For example leaflets in large print.
